Our Basic Direct Cremation Service Charge of $995.00 Includes:
- Transfer of the deceased to our office from a Hospital or County Medical Examiner Office in the Following areas: Westchester County, NYC 5 Boroughs - Bronx, Brooklyn, Queens, Manhattan, Staten Island. Response to Long Island Nassau & Suffolk, Rockland and Putnam Counties are billed by distance from our Yonkers, New York Location. Deaths at a private residence or at a facility requiring an immediate, timely response will carry an additional response fee of $200.00.
- Sheltering the deceased at our facility in a temperature-controlled environment. Embalming is not required by law.
- Obtaining a Certificate of Death from the Medical Examiner or the attending physician, completing the form with information provided to us by you about the deceased, and forwarding the completed form to NYC Dept. of Health OR Registrar of Vital Statistics.
- Providing the party responsible for the Deceased's affairs with all necessary authorization forms and pricing disclosures. These documents can be completed in person at our office, or sent to you via e-mail, or express mail.
- Cardboard alternative container - A cardboard box the body will be placed into at our office and is required by the crematory facility prior to their acceptance from us.
- Transfer of the deceased, in the cardboard alternative container, via our service vehicle to the crematory facility either at Ferncliff Cemetery, Hartsdale, Westchester County, or The Woodlawn Cemetery, Bronx, New York City.
The following MUST BE ADDED to complete service arrangements:
- Crematory Fee - for the cremation process. City Funeral Service engages both The Woodlawn Cemetery in The Bronx ($512.00) and Ferncliff Cemetery in Hartsdale in Westchester County, New York ($496.00). Woodlawn's charges include mailing of the Cremains to you while if you prefer we engage Ferncliff, cremains can be claimed at our Yonkers, NY office
- Cremation permit issued by NYC Department of Health and Mental Hygiene - $40.00 or $20.00 when issued for a death elsewhere in New York State.
- Copies of the Certificate of Death - NOTE: We do not print, issue, store, or archive Death Certificates. We simply obtain a form the Deceased's attending physician, or medical examiner. The form is then completed by our office, and forwarded to the appropriate municipal authority or Department of Health. We order copies on your behalf, and you are not obligated to order any. However, we recommend ordering at least 1 copy for your records, but you may need more depending on how much the deceased has in assets such as bank accounts, life insurance, real estate, retirement funds or other financial instruments. Death certificates are $15.00 Each when a death originiates in the 5 Boroughs of New York City, along with a $23.75 Shipping & Handling Fee, and $10.00 each for locations in New York State outside of NYC.
